Job summary

Haven Hospice in Jacksonville, FL is seeking a Hospice Team Coordinator to provide essential administrative support to clinical leadership and care teams. The role focuses on maintaining organized patient records, handling scheduling, and ensuring smooth coordination across the care team.

Qualified candidates will have at least two years of administrative healthcare experience, preferably in hospice or home health operations, and be adept with office technology and HIPAA/compliance practices.

• Input data into electronic systems (e.g., HCHB) and set up clinician tablets.
• Assist with scheduling, admissions, discharge, and team coordination.
• Support HR functions like maintaining employee records.
• Ensure compliance with hospice policies and Medicare regulations.
