Job summary

Haven Home Health & Hospice in Springfield, MO is seeking an experienced LPN to provide skilled nursing care in patients’ homes, supporting the established plan of care. You will assess, document, and monitor care while working closely with the RN and interdisciplinary team to ensure compassionate, clinically sound service in the place patients call home.

Qualifications

• Current Missouri LPN license.
• At least 1 year of recent LPN experience (acute, long-term care, or home health).
• Home health experience preferred.

• Support and carry out the patient’s plan of care alongside the RN.

Job Summary:

The Licensed Practical Nurse (LPN) provides skilled nursing services to patients in their homes, supporting the established plan of care. You will assess, implement, document, and monitor patient care while working closely with the RN and interdisciplinary team. Your role is essential in ensuring patients receive consistent, compassionate, and clinically sound care in the place they call home.
