Job summary

Haven Home Health and Hospice is seeking a dedicated RN Case Manager to deliver skilled nursing visits and coordinate patient care across the OKC Metro area. The role is based in Norman, OK, with travel required to serve patients in various settings.

The ideal candidate holds an Oklahoma RN license, has 1 year of home health experience, and demonstrates strong assessment, communication, and EMR skills. A valid driver’s license and reliable transportation are required.

Qualifications

• Current RN license in Oklahoma (Required).
• Minimum 1 year of Home Health experience (Preferred).
• Strong assessment and communication skills.
• Experience with electronic medical records preferred.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Ability to travel in varying weather conditions.

Responsibilities

• Deliver skilled nursing visits to assigned patients per physician-ordered plan of care.
• Develop and regularly update individualized care plans.
• Complete and document comprehensive assessments: SOC, ROC, Recertification, Discharge.
• Supervise and support Home Health Aides.
• Educate patients and caregivers on disease processes, medications, and rehab goals.
• Attend and lead weekly case conferences for care planning.
• Coordinate care with physicians and interdisciplinary team members.
• Report changes in patient condition promptly.
• Ensure timely, complete documentation aligning with Medicare and agency guidelines.
• Administer medications and treatments as prescribed.
• Participate in on-call, holiday, and weekend rotations.
• Assist with orientation and training of new staff.
