Job summary

AppleGate Recovery, a BayMark Health Services division, seeks a Full Time Clinical Coordinator to oversee quality care and patient treatment paths in a busy outpatient setting. Responsibilities

• Ensure the maintenance of quality care.
• Monitor treatment of all patients.
• Maintain accurate and current clinical records.
• Conduct internal program review and educational counseling.
• Assess training needs with the Program Director and report them.
• Ensure staff compliance with Federal and State regulations.
• Attend conferences, meetings and trainings as needed.
• Provide intake/readmission services.
• Provide follow‑up calls to discharged patients for 30 days.
• Provide crisis intervention/support to patients as needed.
• Produce interagency reports/documentation as required.
• Perform monthly chart reviews for quality control.
• Participate in community and public relations activities including advertising and marketing.
