Austin Community Court, City of Austin, seeks a Clinical Case Manager to provide assessment and case management for targeted clients. The role includes screening, diagnosis, and development of treatment plans, plus coordination with community services.

Responsibilities include delivering individual therapy, documenting data, and maintaining safe, compliant operations within the facility, while collaborating with City staff and external agencies.

Formación

• Must possess knowledge of DSM-5 and assessment practices.
• Experience with case management and documenting client data.
• Ability to engage clients and maintain boundaries in high-stress settings.

Responsabilidades

• Conducts screening and assessment to determine diagnosis and treatment of mental, emotional, and behavioral disorders, conditions, and addictions; implements and reviews individual treatment plans based on best practice.
• Assists client in the stabilization of bio-psychosocial functioning.
• Provides individual therapy.
• Provides case management, keeps up-to-date documentation, compiles data and informational reports, and prepares cost estimates.
• Refers citizens to appropriate agency or program.
• Records data on appropriate form and/or log.
• Attends meetings and/or seminars.
• Maintains safety and security of self, others, and facility.
