Job summary

Maimonides Medical Center is seeking a clinical social worker to join the psychiatry inpatient social work team. You will develop, plan, coordinate utilization management, discharge planning, and case management across the patient care continuum, collaborating with nursing, physicians, and other clinical staff to optimize patient outcomes.

The role requires an LMSW with at least 3 years of direct experience in utilization management and discharge planning, along with strong communication skills

Qualifications

• LMSW required; LCSW preferred.
• Minimum 3 years in utilization management, discharge planning or case management.
• Excellent oral and written communication skills.
• Knowledge of rules and regulations of third party payor and regulatory agencies.
• English proficiency; second language preferred.

Responsibilities

• Coordinate discharge planning and case management with the clinical team.
• Collaborate with physicians and insurers to optimize benefits.
• Lead interdisciplinary rounds and reviews to ensure appropriate care.
