Job summary

The ideal candidate has NYS RN licensure, BSN (MSN preferred), 3–5 years in clinical settings, and leadership experience in healthcare.

Qualifications

• NYS RN licensure required.
• BSN required; MSN preferred.
• 3–5 years clinical experience in Med/Surg acute care.
• CCM preferred.
• Minimum 3 years leadership or managerial experience in healthcare.
• Proficiency with case management software and EMRs (Allscripts, Sunrise) preferred.
• Strong communication and problem-solving skills.

Responsibilities

• Direct, supervise and manage the Case Management Team.
• Facilitate goals of reducing length of stay and improving patient care.
• Ensure efficient and effective utilization of resources for appropriate continuum of care.
• Act as liaison with nursing and medical staff and other clinical departments.
