Job summary

Maimonides Medical Center is seeking a Manager of the Case Management Team to lead day-to-day operations and drive efficient patient care processes in a large hospital system.

The role focuses on reducing length of stay, coordinating discharge planning, and collaborating with nursing and other clinical departments to ensure seamless transitions for patients across the continuum of care.

Qualifications

• 3-5 years clinical experience (Med/Surg acute care) required.
• Minimum 3 years direct experience in utilization management, discharge planning, case management or home care.
• Minimum 2 years leadership or managerial experience in a healthcare environment preferred.
• Proficiency with case management software and EMRs (e.g., Allscripts, Sunrise, or equivalent) preferred.
• Excellent oral and written communication skills.

Responsibilities

• Oversee day-to-day directing, supervising and managing the Case Management Team.
• Facilitate hospital goals: reduce length of stay, improve patient care and efficient resource use.
• Serve as liaison with nursing, medical staff and clinical departments to coordinate care.
• Provide guidance on discharge planning and barrier removal to ensure smooth transitions.
