RHA Health Services seeks a Clinical Case Manager to support individuals with intellectual and developmental disabilities and mental health/substance use challenges. The role oversees ISP development, implementation, and progress, coordinating with residential teams, nurses, and care staff.

The position requires a social services degree, 2+ years of IDD experience, and strong regulatory knowledge. This onsite role in New Jersey offers comprehensive benefits and growth opportunities.

Competenze

• Bachelor's degree in Social Work, Social Services, Human Services or Special Education is required (Master's degree a plus).
• Minimum 2 years experience with individuals with intellectual/developmental disabilities; 1 year if Master's degree.
• Valid driver's license, auto insurance, and reliable transportation.
• Working knowledge of regulations applicable to services.
• Must be at least 18 years old.

Mansioni

• Coordinate IDT meetings and ISPs.
• Supervise ISP implementation and progress.
• Ensure a streamlined admission process.
• Respond to incident reports and perform root cause analysis.
• Communicate with the Support Coordinator and Circle of Support for ISP amendments.
• Document all communications.
• Spend time at service locations and conduct visits to ensure policy adherence.
• Provide coverage when direct care staff are short while maintaining management duties.
• Participate in on-call rotation including weekends and holidays.
• Train and conduct in-services for direct care staff.
• Conduct internal QA reviews and Personal Outcome Measures.
