Salary and Benefits: $63,000 annually; mileage reimbursement

About Home Study and Post Release Services
KyUMH works in collaboration with the Office of Refugee Resettlement (ORR) to support children who are placed with caregivers across Kentucky and potentially neighboring states. The goals of the Home-Study Post Release Services Program (HS/PRS) are to:

Job Specific Duties and Responsibilities
The Caminos Post Release Services (PRS) Clinical Case Manager (CCM) provides community-based individual and family therapy to recently migrated children, adolescents, and their families.
The CCM is responsible for delivering culturally competent services including mental health assessments, screening for human trafficking concerns, crisis intervention, support services, and psychoeducational support groups for recently migrated youth adjusting to life in the U.S.
The CCM supports KYUMH’s trauma-responsive model by collaborating with the post-release team to ensure comprehensive services and connection to community resources.
Ensures all assessments and interventions are culturally sensitive and involve youth, families, and sponsors appropriately.
Expected Hours of Work
This is a full-time position requiring flexibility, including evenings and weekends, based on youth and family availability.
Travel
Travel is required, including local, out-of-area, and overnight trips. Must be willing to fly, drive, and stay overnight as needed.
