About Home Study and Post Release Services

KyUMH works in collaboration with the Office of Refugee Resettlement (ORR) to support children who are placed with caregivers across Kentucky and potentially neighboring states. The goals of the Home-Study Post Release Services Program (HS/PRS) are to:

  • Assess the safety and suitability of a sponsor before an unaccompanied child is released into a sponsor’s care (Home Study).
  • Provide case management support to ensure the continued safety and stability of children in sponsor’s homes and link children to community resources (Post Release Services).

Required Education and Experience

  • Current LPCA, LPCC, MSW, LCSW, LMFTA, LMFT, LMHC OR eligible for licensure at the time of application.
  • Master’s Degree in Social Work, Psychology, Sociology, Counseling, or relevant behavioral science with direct clinical experience OR
  • Bachelor’s degree in one of the above fields from an accredited college or university with at least five (5) years of clinical experience
  • Fluency in both English and Spanish, oral and written

Preferred Education and Experience

  • Experience working with diverse populations, including refugee and/or unaccompanied immigrant children and adolescents
  • One-year experience providing direct services to youth and families
  • One-year experience delivering community-based and/or in-home services

Job Specific Duties and Responsibilities

The Caminos Post Release Services (PRS) Clinical Case Manager (CCM) provides community-based individual and family therapy to recently migrated children, adolescents, and their families.

The CCM is responsible for delivering culturally competent services including mental health assessments, screening for human trafficking concerns, crisis intervention, support services, and psychoeducational support groups for recently migrated youth adjusting to life in the U.S.

The CCM supports KYUMH’s trauma-responsive model by collaborating with the post-release team to ensure comprehensive services and connection to community resources.

  • Conducts in-person visits and clinical meetings with clients in their homes weekly for at least the first 30 days, or longer as needed.
  • Maintains regular monthly contacts with clients for at least 6 months after weekly services end.
  • Provides in-person support during stressful or traumatic experiences, including attending medical or psychiatric appointments, social service meetings, educational meetings, or hospital visits.
  • Documents all contacts, resources, referrals, clinical notes, interventions, and assessments in accordance with KYUMH and ORR deadlines.
  • Develops and implements culturally competent interventions to help youth process trauma and adapt to life in the U.S.
  • Ensures youth are supported to discuss concerns related to their migration journey and life in their home country.
  • Collaborates with case management to address mental health needs, modify behaviors, and support healing and social skills development.
  • Conducts screenings and assessments for special needs, medical or mental health concerns, criminal history, gang involvement, and trauma experiences within mandated timeframes.
  • Performs child trafficking assessments and collaborates on referrals to anti-trafficking resources.

Ensures all assessments and interventions are culturally sensitive and involve youth, families, and sponsors appropriately.

Expected Hours of Work

This is a full-time position requiring flexibility, including evenings and weekends, based on youth and family availability.

Travel

Travel is required, including local, out-of-area, and overnight trips. Must be willing to fly, drive, and stay overnight as needed.
