Job Details

Location: Remote (Must reside in Orlando, FL) with travel to Orange, Osceola, Brevard, & Seminole Counties. Travel up to 75%. Hours: M - F, 8am - 5pm EST, with some flexibility.

Pay: $38/hr, bi-weekly.

Responsibilities
  • Complete at least 10 NF member assessments weekly.
  • Support the Health Plan Rebalancing Initiative by assessing, identifying, screening, and transitioning NH members into the community.
  • Follow up on case management referrals and visit members in-person twice a week.
  • Conduct assessments prior to discharge, identify environmental supports needed, and coordinate services.
  • Perform in-person Significant Change Visits within 5 days of transition.
  • Coordinate services, establish Plans of Care, and document actions.
  • Coordinate with facilities, social services, and case managers to facilitate discharges and transitions.
  • Identify high-risk community members and implement interventions to prevent lapses or ensure safe transitions.
  • Engage in community relations events to enhance member satisfaction and retention.
Qualifications
  • Minimum 2 years of clinical experience.
  • Bi-lingual in Spanish and English.
  • Willing to travel up to 75%.
  • Preferred: Managed care, discharge coordination, transition of care, home health, and case management experience.
  • Proficiency with computer systems and ability to work in a fast-paced environment.
