Job Details

• Title: Bilingual RN Case Manager
• Pay: $38/hr, Bi-weekly
• Type: Full-time, Remote (Must live in Tampa, FL)
• Shift: M-F 8am-5pm EST with some flexibility

Responsibilities

• Service Hillsborough, Highlands, Polk, Hardee, and Manatee counties, with up to 75% local travel.
• Assess, identify, and support NH members transitioning into the community.
• Follow up on referrals, conduct assessments, and coordinate services.
• Engage in community events to enhance member satisfaction and retention.

Qualifications

• Minimum 2 years of clinical experience.
• Bi-lingual Spanish/English required.
• Willing to travel 75% of the time.
• Preferred: Managed Care, Discharge Coordination, Transition of Care, Home Health, and Case Management experience.
