Qualifications

  • Master’s Degree in Behavioral/Mental Health or related field.
  • LCSW or LCPC in Illinois.
  • 3-5 years clinical practical experience.
  • 2-3 years CM, discharge planning and/or home health care coordination experience.
  • Must reside in Illinois.
  • Must possess reliable transportation.
  • Willing and able to travel up to 50-75% in Southloop, Chinatown and surrounding areas.
  • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications.
  • Excellent analytical and problem-solving skills.
  • Effective communications, organizational, and interpersonal skills.
  • Ability to work independently.

Responsibilities

  • Conduct comprehensive assessments of referred members’ needs and eligibility.
  • Determine case-resolution approaches using benefit plans and internal/external programs and services.
  • Apply clinical judgment and strategies to reduce risk factors and address complex clinical indicators.
  • Perform crisis intervention for members experiencing behavioral health or medical crises.
  • Refer members to appropriate clinical providers and provide crisis follow-up.
  • Apply clinical criteria, guidelines, case-management plans, policies, procedures, and regulatory standards.
  • Consult with supervisors, Medical Directors, and other programs to overcome care barriers.
  • Present cases at case conferences for multidisciplinary review.
  • Identify and escalates quality-of-care issues.
  • Influence appropriate member care and promote member engagement and lifestyle changes.
  • Provide coaching, information, and support for independent healthcare decisions.
  • Help members participate knowledgeably in healthcare decision-making.
  • Analyze utilization, self-report, and clinical data to identify comprehensive member needs.
  • Develop and monitor plans of care with members and care teams.
  • Use case-management and quality-management processes in compliance with regulatory, accreditation, and company requirements.

Requirements

  • Ability to work independently

  • Master’s Degree in Behavioral/Mental Health or related field
  • LCSW or LCPC in the state of Illinois
