Responsibilities

  • Perform psychosocial assessments and assess patients' medical history, mental health history, and current presentation.
  • Develop, implement, and modify individualized plans of care.
  • Assess crisis situations and provide needed resources or referrals.
  • Diagnose and treat mental, emotional, and behavioral disorders.
  • Provide family and individual treatment, supportive interventions, crisis and bereavement support, and other social work interventions.
  • Provide social work consultation for patients and caregivers.
  • Organize and prioritize daily work according to patient needs.
  • Refer patients to community services, treatment programs, support groups, psychiatrists, and other mental health providers.
  • Educate, communicate, and coordinate with patient care teams and outside agencies.
  • Support quality care, timely discharge, and resolution of delays and issues.
  • Advocate for patients and families and facilitate effective multidisciplinary communication.
  • Develop discharge plans and coordinate continuity of care.
  • Complete timely and accurate medical-record documentation in accordance with clinical and billing standards.
  • Identify patients at risk for avoidable delays, extended stay, readmission, financial needs, or complex discharge needs.
  • Coordinate with care providers and leaders to reach optimal solutions.

Requirements
  • REQUIRED: LCSW
  • Master's Degree in Social Work from an accredited institution; degree will be verified
  • Current State Licensure as LCSW or LMFT
  • Computer skills, including email and word processing
  • Excellent written and verbal communication skills
  • Selected candidate must successfully complete a 60 day credentialing process before starting
  • Experience with patients, families, and treatment teams preferred
  • Ability to see and read information, labels, and assess patient needs
  • Ability to verbally communicate and hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergencies
  • Manual dexterity to manipulate complex and delicate equipment with precision and accuracy
  • Frequent computer use and typing for documenting patient care and accessing information
  • Ability to walk, assist with transporting/ambulating patients, and obtain and distribute supplies and equipment
  • Ability to push/pull gurneys and portable equipment, including heavy items
  • Ability to stand stationary for an extended period
  • Compliance with federal, state, local, and clinical program/facility standards
  • Acts within the scope of the Mental Health Practice Act
