Job summary

Option Care seeks a licensed RN or Pharmacist to manage referral-source relationships and educate patients and families about discharge services. You will coordinate care transitions and work with hospitals to ensure smooth transitions to home or alternate-site care.

The role includes travel to partner facilities to deliver live education and training, collaborate with Account Managers, and uphold ACHC/URAC standards while delivering high-quality patient care.

Qualifications

Job description

  • Maintain and grow referral-source relationships to increase sales and patient starts
  • Serve as Option Care’s key contact and representative for referral-source education, assistance, and service
  • Interact with area service providers to sell Option Care services for discharging patients
  • Assess patients selected by hospitals to determine viability for alternative-site care
  • Educate patients, families, and facility staff about Option Care services and products
  • Provide hands-on, in-person education to patients and caregivers to support timely discharge and therapy independence
  • Communicate with agency staff, medical teams, patients, and families throughout discharge to implement treatment plans
  • Collect, review, complete, and secure signoff for pre-admission information
  • Partner with the Account Manager and Regional Sales Director on area business and growth plans
  • Discuss opportunities and progress with the Account Manager and provide feedback on promoting Option Care services
  • Deliver educational and promotional programs with Account Manager and marketing staff
  • Review medical records for medical history and primary/secondary insurance information and communicate it to patient registration
  • Coordinate care transitions with referral sources and internal partners for seamless transitions to home or ATS
  • Support data collection, therapy starts, patient tracking, and transfer of orders by phone, fax, and e-prescribing
  • Maintain confidentiality of patient and proprietary information and follow legal safeguards
  • Provide oversight and input regarding patient assessment, treatment, and transition to home care
  • Coordinate communication with other providers
  • Arrange special medical supplies or appliances
  • Travel to partnership facilities, community hospitals, and medical practice offices to sell services, process referrals, and provide live education and training
Requirements
  • Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice
  • At least 2 years of experience in the healthcare industry
  • Experience establishing and maintaining relationships with individuals at all levels of the organization, in the business community, and with vendors
  • Experience applying knowledge of standard practices for all services offered and current relevant and applicable standards, including ACHC and URAC standards
  • Experience providing customer service to internal and external customers, including meeting quality standards and evaluating customer satisfaction
  • Basic PC skills and competent use of internet, email, Microsoft Word, Microsoft Excel, and Microsoft PowerPoint
  • Experience identifying operational issues and recommending and implementing strategies to resolve and improve processes
  • Reliable transportation for travel to care facilities, home visits, and multiple hospitals
  • Valid driver’s license and proof of insurance if using a personal vehicle
  • Ability to plan, organize, and make presentations
  • Hands-on home or alternate site infusion or discharge planning experience preferred
  • Experience growing service provider partnerships preferred
