Job summary

Option Care seeks a healthcare navigator to strengthen referral relationships, educate patients and staff, and coordinate discharge planning across facilities. The role focuses on assessing patient suitability for alternative-site care and ensuring smooth transitions to home or other settings.

You will educate patients, families, and facility staff, manage orders and transfers, and collaborate with account managers to execute area growth plans while maintaining confidentiality and compliance

Qualifications

  • RN or Pharmacist licensed in state of practice.
  • Minimum 2 years in the healthcare industry.
  • Experience building relationships across organizations and vendors.
  • Familiarity with ACHC and URAC standards.
  • Strong customer service skills for internal/external clients.
  • Proficient with MS Office tools (Word/Excel/PowerPoint).
  • Travel to care facilities, home visits, and multiple hospitals as needed.
  • Valid driver’s license and proof of insurance if using personal vehicle.
  • Ability to plan, organize, and present effectively.
  • Home or alternate site infusion or discharge planning experience preferred.

Responsibilities

  • Proactively maintain and grow referral-source relationships to increase sales and patient starts.
  • Serve as a key point of contact for education, assistance, and service to referral sources.
  • Interact with area providers to sell services for discharging patients.
  • Assess patients selected by hospitals for suitability of alternative-site care.
  • Educate patients, families, and facility staff about Option Care services.
  • Provide hands-on, in-person education to patients and caregivers for timely discharge.
  • Coordinate discharge process with agency staff, medical teams, and families.
  • Collect and secure signoff for pre-admission information.
  • Partner with Account Managers and Regional Director to execute growth plans.
  • Discuss opportunities, provide feedback to Account Manager, and align with marketing.
  • Review medical records for pertinent history and insurance information for patient registration.
  • Coordinate care transitions to home or alternate sites.
  • Facilitate order transfers by phone/fax/e-prescribing and participate in data collection.
  • Maintain confidentiality and comply with legal safeguards.
  • Provide input to providers regarding patient assessment and transition to home care.
  • Serve as a contact and coordinator with other providers.
  • Arrange special medical supplies or appliances.

Job description

  • Proactively maintain and grow referral-source relationships to increase sales and patient starts
  • Serve as a key point of contact and representative of Option Care, providing education, assistance, and service to referral sources
  • Interact with area service providers to sell Option Care services for discharging patients
  • Assess patients selected by hospitals to determine suitability for alternative-site care
  • Educate patients, families, and facility staff about Option Care services and products
  • Provide hands‑on, in‑person education to patients and caregivers to support timely discharge and therapy independence
  • Communicate with agency staff, medical teams, patients, and families throughout the discharge process
  • Collect, review, complete, and secure signoff for pre‑admission information
  • Partner with Account Managers and the Regional Sales Director to create and execute area business and growth plans
  • Discuss opportunities, assess progress, and provide feedback to the Account Manager
  • Partner with Account Managers and marketing staff to deliver educational and promotional programs
  • Review medical records for pertinent history and insurance information and communicate it to patient registration
  • Coordinate care transitions with referral sources and internal partners to support seamless transitions to home or alternate treatment sites
  • Facilitate order transfers by phone, fax, and e‑prescribing, and participate in therapy‑start and patient‑tracking data collection
  • Maintain confidentiality of patient and proprietary information and comply with legal safeguards
  • Provide oversight and input to providers regarding patient assessment, treatment, and transition to home care
  • Serve as a point of contact, coordination, and communication with other providers
  • Arrange special medical supplies or appliances
Requirements
  • Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice
  • At least 2 years of experience in the healthcare industry
  • Experience establishing and maintaining relationships with individuals at all levels of the organization in the business community and with vendors
  • Experience applying knowledge of standard practices for all services offered and current relevant and applicable standards, including ACHC and URAC standards
  • Experience providing customer service to internal and external customers, including meeting quality standards and evaluating customer satisfaction
  • Basic PC skills, including internet, email, Microsoft Word, Microsoft Excel, and Microsoft PowerPoint
  • Experience identifying operational issues and recommending and implementing strategies to resolve and improve processes
  • Access to reliable transportation for travel to care facilities, home visits, and multiple hospitals
  • Valid driver’s license and proof of insurance required if using a personal vehicle
  • Able to plan, organize, and make presentations
  • Hands‑on home or alternate site infusion or discharge planning experience preferred
  • Experience growing service provider partnerships preferred
