Job summary

New York Cancer & Blood Specialists (NYCBS) seeks an LPN in Bohemia, NY to support oncology patients. You will monitor communications, educate patients, coordinate appointments, and assist with FMLA paperwork and medication refills.

Strong nursing skills and NY licensure are required, with one year of experience preferred. You will work with physicians and nurses to implement care plans, coordinate chronic care management, and ensure timely authorizations and referrals.

Qualifications

  • Graduate from an accredited School of Nursing with an unencumbered New York license.
  • Nursing experience with one year of clinical competence preferred.
  • Excellent assessment, communication and organizational skills.
  • Good computer knowledge.
  • Ability to plan, organize, develop, and interpret health data, goals, and objectives for quality care coordination.
  • Urology experience preferred.

Responsibilities

  • Monitor and manage patient communications and ensure timely responses for care coordination.
  • Provide patient outreach and education and address clinical questions within LPN scope.
  • Guide patients through their care journey by coordinating appointments and follow-ups.
  • Assist centralized FMLA team with completion of forms requiring provider signature.
  • Process prescription refill requests with healthcare providers for accuracy and timeliness.
  • Use Prescription Monitoring System (PMP) for refill requests of controlled substances with proper follow-up.
  • Liaise with revenue cycle team for authorizations and referrals updates.
  • Arrange Peer to Peer conversations with providers to finalize testing and medication authorizations.
  • Collaborate with physicians, nurses, and other team members to implement individualized care plans.
  • Identify qualifying patients for Chronic Care Management to reduce ER visits.
  • Coordinate care with triage team and APP for acute symptoms.
  • Serve as a liaison between patients and the healthcare team to address concerns.
  • Educate patients to include health needs in overall treatment plans.
  • Maintain up-to-date patient records documenting interactions, care time, and outcomes in compliance with health regulations.
