Job summary

New York Cancer And Blood Specialists (NYCBS) is seeking a registered nurse to join our Bohemia, NY team as a Triage/Chronic Care Management RN. You will split duties between triage and CCM, performing telephonic symptom assessments and guiding patients through care plans.

Responsibilities

• Perform telephonic symptom assessment for patients, practice-wide.
• Utilize triage protocols to determine level of care and urgency.
• Provide patient education on symptom management and treatment side effects.
• Create and revise care plans for newly enrolled patients as clinically appropriate.
• Coordinate care with physicians, APPs, and staff.
• Maintain accurate documentation in compliance with HIPAA and regulations.
• Deliver exceptional customer service with professionalism and empathy in high-volume calls.
