21st Century Home Health Services is seeking a Clinical Supervisor - Nursing to lead RN Case Managers and LVNs, ensuring coordinated in-home patient care. You will balance fieldwork with office oversight, driving clinical excellence and patient outcomes across the Sacramento area.

Ideal candidates will have an active CA RN license, BLS, and 2+ years in management within health care. The role offers a hybrid schedule with a strong focus on team development and performance improvement.

Qualifications

• Two years' management or supervisory experience within the past five years in a health care setting, preferably home health.
• Graduate of an accredited school of nursing; RN license required.
• Current CA Registered Nurse License (RN) and Basic Life Support (BLS) certification.
• Valid CA Driver's License with acceptable driving record.
• Bachelor's Degree in nursing preferred.

Responsibilities

• Oversee field clinicians delivering in-home patient care and coordinate care plans.
• Serve as the first point of contact for field clinicians; escalate issues as needed.
• Coordinate bi-weekly Case Conferences with clinical teams to review progress and adjust care plans.
• Review patient records, clinical diagnoses, and medications to guide care.
• Support ongoing staff development and ensure compliance with clinical standards.
