Job summary

Bay Area Community Services (BACS) is seeking a Clinical Care Coordinator to deliver comprehensive therapeutic interventions and services to adults in wellness and recovery programs. Duties include counseling, case management, housing navigation, and crisis intervention, with after-hours scheduling as needed.

The THRIVE programs target justice-involved adults, offering outpatient services Monday–Friday and 7-day coverage for higher-intensity cases.

Qualifications

• Requires current BBS registration with ASW/AMFT/APCC or LCSW/LMFT/LPCC.
• Valid California driver’s license and own vehicle with insurance; DMV printout required.

Responsibilities

• Deliver individual, family, group counseling and case management.
• Use evidence-based practice to ensure high-quality outcomes.
• Assess risk status and intervene appropriately.
• Create person-centered service plans to help clients improve.
