Qualifications

• Bachelor’s degree in social work, nursing, psychology, public health, or related field.
• Current clinical license preferred (e.g., LCSW, LCPC, RN).
• 3+ years of experience in care coordination or patient navigation.
• 1+ year of supervisory or lead experience.
• Strong knowledge of primary care integration and social drivers of health.
• Experience with EHR systems and data dashboards.

Responsibilities

• Supervise and coach a team of care coordinators and patient navigators.
• Oversee outreach, appointment reminders, and care coordination across services.
• Help address social drivers of health and link to resources.
• Develop patient-centered care coordination plans with documentation.
• Refine workflows for patient navigation with leaders.
• Monitor worklists and population-health tools for outreach and follow-up.
• Review metrics and drive quality-improvement initiatives.
• Provide training and coaching on communication, trauma-informed care, and documentation.
• Collaborate with clinical leaders to delineate responsibilities.
• Ensure privacy and confidentiality, especially for behavioral health data.
• Participate in hiring and performance evaluations.
