Canyon Home Care & Hospice LLC is seeking a Clinical Manager to assist the Administrator with supervision of home care and hospice services for patients in their residences, ensuring the highest quality of care. The role requires availability during operating hours and active participation in professional service activities.

The manager coordinates patient care and referrals, develops the plan of care, ensures compliance with Medicare/Medicaid and other regulations, and supports QAPI

Competenze

• Licensed professionals in relevant fields per state.
• At least one year supervisory experience in home care or related health program.
• Physician, registered nurse, or a health care professional licensed in Utah to provide supervision and direction for professional services.

• Adheres to all applicable regulations, standards, and laws; follows agency policies.
• Supervises and develops patient care personnel skills and performance.
• Assists Administrator with staffing, recruiting, hiring, evaluating and disciplinary actions.
