Job summary

Concierge Home Care in Valrico, FL, is seeking a Clinical Care Coordinator, RN, to support our clinical team. The role is primarily office-based with a Monday–Friday schedule, offering leadership over home health clinicians and a focus on quality patient care.

The ideal candidate holds an active Florida RN license with 1+ year of home health experience and strong coordination and communication skills. Travel locally to support operations is expected.

Qualifications

• Active Florida or Compact RN license is required.
• Minimum 1 year RN experience in acute or post-acute care.
• At least 1 year of skilled home health experience.
• Strong clinical judgement and communication skills.

Responsibilities

• Coordinate day-to-day clinical operations of a home health team.
• Provide guidance, support, and coaching to clinicians.
• Review clinical documentation for Medicare and regulatory compliance.
• Monitor quality of care and patient outcomes.
• Collaborate with physicians and interdisciplinary team members.
• Coach and mentor clinicians for professional development.
• Ensure adherence to agency policies and clinical standards.

Are you an experienced Registered Nurse with home health experience who enjoys coordinating care, supporting clinicians, and helping ensure patients receive high-quality care? Concierge Home Care is looking for a Clinical Care Coordinator, RN to support the clinical team.

This is an opportunity to use your clinical expertise in a Monday–Friday, primarily office-based role while helping clinicians succeed and ensuring patients receive safe, compliant, high-quality home health services.

What You'll Do

• Support and coordinate the day-to-day clinical operations of a home health team
• Provide guidance, support, and coaching to home health clinicians
• Review clinical documentation for Medicare and regulatory compliance
• Monitor quality of care, patient outcomes, and clinical performance
• Collaborate with physicians and interdisciplinary team members
• Help clinicians navigate clinical and documentation challenges
• Coach and mentor clinicians to support professional development
• Ensure agency policies, procedures, and clinical standards are consistently followed

Schedule

• Monday–Friday
• Primarily office-based
• Local travel as needed to support clinical operations
• Leadership and clinical coordination responsibilities without a traditional bedside schedule
• Three weeks PTO, increasing to four weeks after five years
• PTO accrual begins the first day of employment
• Quarterly performance bonuses

Qualifications

• Active, unencumbered Florida or Compact RN license
• Minimum 1 year of RN experience in acute ot post-acute care
• At least 1 recent year of skilled home health experience
• Strong clinical judgment and communication skills
• Ability to coach, support, and collaborate with clinical professionals
• Valid driver's license
• Reliable transportation and current auto insurance
• Ability to travel locally as needed
