Job summary

Your Health Organization in the Charleston area seeks a Clinical Coordinator to support clinical operations, chart preparation, scheduling, and care coordination across facilities, homes, clinics, and virtual visits. The role requires a CMA or licensed nurse, CPR, strong communication, and ability to travel between offices; it is a full-time, 8:00 AM–5:00 PM position, Monday–Friday.

Qualifications

• Must be a CMA or licensed nurse in state of employment.
• CPR certificate current; licensed in good standing.
• High school diploma or equivalent.
• Strong interpersonal and communication skills in all conditions.
• Ability to manage multiple responsibilities and demonstrate sound judgment.

Responsibilities

• Review and organize patient records for visits.
• Ensure documentation and test results are accessible for providers.
• Maintain confidentiality and privacy regulations.
• Reconcile medications and coordinate with care team for appointments.
• Communicate with patients via phone, email, portal, text, and in person.
