Job summary

Your Health in West Columbia, SC is seeking a Clinical Coordinator to support clinical needs, prepare charts, schedule visits, and log care management activities. The role requires strong communication and eligibility to travel between offices.

This full-time, 8-hour salary position operates 8:00 AM–5:00 PM, Monday through Friday, within a fast-paced value-based care environment focused on coordinated patient care.

Qualifications

• Must be a CMA or licensed nurse (RN/LPN) in the state of employment.
• Minimum of 1 year experience in a healthcare setting preferred.
• Must hold and maintain current CPR certificate.
• High school diploma or equivalent required.
• Strong interpersonal and communication skills; ability to multitask and work in a team.
• Flexible hours and ability to travel between offices/facilities.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure all necessary documentation and results are accessible for the provider; use Navina to ensure updates.
• Maintain accurate patient records (demographics, contact details, insurance, appointment history).
• Ensure confidentiality and privacy; verify active consent forms and alignment paperwork.
• Reconcile medications.
• Coordinate patient appointments with providers and care team partners.
• Communicate with patients via email, portal, phone, text, and in person to gather information and schedule visits.
• Document and log care management activities.
