Job summary

Your Health in Ladson, SC is seeking a Clinical Coordinator to support all clinical needs for our Charleston location patients. The role involves chart prep, scheduling visits, logging care management activities, and providing provider and patient support.

This full-time, 8-hour salary position runs M-F 8:00 AM–5:00 PM. You will travel between offices as needed and collaborate with the care team to ensure coordinated, high-quality patient care in a fast-paced, value-based environment.

Qualifications

• Certified Medical Assistant (CMA) or licensed nurse (LPN/RN) in good standing.
• At least 1 year healthcare experience preferred.
• Current CPR certification.
• High school diploma or equivalent.
• Strong interpersonal and communication skills.
• Ability to collaborate with care teams and manage multiple tasks.
• Basic computer literacy.
• Willingness to travel between offices and have a valid driver’s license.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure documentation, test results, diagnoses, and information are accessible for the provider; use Navina to update records.
• Maintain confidentiality and privacy compliance; verify active consent forms and alignment paperwork.
• Reconcile medications.
• Coordinate patient appointments with providers and care team.
• Interact with patients via email, portal, phone, text, and in person to gather information and provide instructions.
• Document and log care management activities accurately.

Education

CMA or LPN/RN license in good standing
