The Valley Hospital-Paramus is seeking a social worker to coordinate and facilitate patient care within a multidisciplinary team across the Valley Hospital Health Care System. The role focuses on patient progression, discharge planning, and effective use of health-system resources.

Requirements include a Master’s in Social Work, New Jersey social work licensure (MSW with LSW or LCSW), with LCSW preferred, and a minimum of three years of social work experience in a healthcare setting.

Qualifications

• Master’s degree in social work from an accredited program.
• New Jersey Social Work License required.
• MSW and LSW or LCSW required.
• LCSW strongly preferred.

Responsibilities

• Coordinate and facilitate patient care through the Valley Hospital Health Care System as part of a multidisciplinary approach to patient progression, discharge planning and efficient utilization of resources.
• Collaborate with physicians, rehab, respiratory, DI, Lab, and other team members to ensure seamless care and appropriate disposition.
