Job summary

CenterWell Home Health is seeking a Clinical Liaison/Patient Care Coordinator to join our local sales team and coordinate patient care across hospitals and SNFs in the assigned territory. You will meet patients at their bedside to discuss home health services, and ensure a smooth start of care with the operations teams.

You may assist with sales calls to support the Account Executive in addressing client needs and business objectives.

Qualifications

• LPN, LVN or RN license is required.
• Knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
• Three Years Clinical Experience Preferred.

Responsibilities

• Coordinate care from hospitals and SNFs in the territory and meet with patients at their bedside to discuss home health services and expectations.
• Support operations by ensuring resources, information, and coordination for a successful start of care.
• Assist with sales calls as needed to support Account Executive in addressing client needs and advancing business objectives.
