Job summary

St Davids Care Partners, part of the HCA Healthcare family, seeks an Administrative Care Coordinator to ensure high-quality care coordination within a Clinically Integrated Network (CIN). You will monitor progress toward network goals, support care navigation, and collaborate with providers and payers to improve outcomes.

The role involves outreach to high/medium risk patients, patient education, and contributing to cost savings and process improvement across the CIN.

Qualifications

• Healthcare experience is preferred.
• Knowledge of primary care practice process preferred.
• Experience evaluating and monitoring data around quality metrics preferred.
• Strong computer skills, particularly related to Microsoft applications like Word, Excel, PowerPoint and Outlook.
• Working knowledge of health information technology.

Responsibilities

• Coordinate care within the CIN and monitor progress toward network goals and quality outcomes.
• Conduct outreach and follow up regarding high/medium risk patients and gaps in care.
• Provide patient education on navigating the health care system and community resources.
• Collaborate with providers, practice teams and payer coordinators to address patient needs and barriers.
• Support cost of care savings and improved health outcomes through CIN initiatives.
• Test and pilot new value-based care initiatives and refine processes with leadership.
• Review payer reports and interpret actions needed for care coordination.
• Assist in developing and monitoring CIN policies and procedures related to value-based programs.
• Communicate clearly with all levels to foster collaboration and accountability.
• Perform other duties as assigned by supervisor.

Job Summary and Qualifications

As the Clinical Care Coordinator, you will be accountable for ensuring a high standard of quality and coordination of care in support of a Clinically Integrated Network (CIN). You willmonitorprogress toward network goals, quality care gap management, transitions of care foridentifiedpopulations, and care coordination to drive quality and cost outcomes for the attributed populations. You will support the CIN care coordination/care management initiatives, which may include outreaching, triaging, and providing care navigation for patients attributed to the CIN.

• Conduct outreach and follow up regarding patients that are high and medium risk and gaps in care.
• Provide patient education relative to navigating the health care system and community resources.
• Collaborate with providers and practice teams to communicate patient needs and develop solutions to overcome barriers.
• Collaborate with the payer systems and payer Care Coordinators on specific patient care needs to include encouraging them to work with their applicable payer Case Manager, Disease Manager or Wellness Program designee.
• Supports the efforts of providers and practice teams on office-based care delivery interventions resulting in cost of care savings and improved health outcomes for patients.
• Test and pilot new VBC initiatives and work with leadership on refining processes, helping the CIN achieve contractual metrics.
• Review reports from payers and assist Director of Clinical Operations in the interpretation of the reports and actions needed.
• Assist in the development, implementation and monitoring of CIN policies and procedures as they relate to value-based programs.
• Communicates clearly, regularly and effectively with all levels and functions to create an atmosphere of mutual respect, openness, and encouragement.
• When the need arises, will perform other duties as assigned by supervisor.

What qualifications you will need:

• Healthcare experience is preferred.
