Job summary

AdventHealth in Orlando, FL is seeking a Nursing role focused on hepatology care to support complex patient needs within the Hepatology team. You will coordinate with liver transplant partners and community providers to optimize outcomes, while guiding patients through insurance processes and required forms.

The role requires strong clinical judgement, excellent communication, and the ability to work both independently and as part of a team.

Qualifications

• Bachelor's degree in a health-related field preferred.
• Three years nursing experience in inpatient and/or outpatient required.
• Experience with care of liver/GI disease and organ failure preferred.
• Outpatient patient care management experience preferred.
• Teaching/preceptor experience preferred.

Responsibilities

• Supports complex patient care issues and proposes proactive care plans.
• Acts as liaison with Hepatology patient population, Liver Transplant Coordinator, and community care providers.
• Maintains knowledge of evidence-based practices and advocates for Hepatology patients as part of the team.
• Assists patients with non-medical needs related to care insurance/referral processes and forms.
• Other duties as assigned.

Job Description:

• Supports complex patient care issues, identifies patients at risk for suboptimal outcomes, and proposes proactive care plans.
• Acts as a liaison with the Hepatology patient population, the Liver Transplant Coordinator, and community care providers.
• Maintains knowledge of current evidence-based practices in area of expertise. Advocates for Hepatology patients as an active member of the Hepatology team, including advanced assessment, planning, coordination of care, modification of treatment plans, and monitoring and evaluation of outcomes.
• Assists and guides patients with non-medical needs related to care insurance/referral processes, pharmaceutical forms and prescriptions, and disability/FMLA forms.
• Other duties as assigned.
