AdventHealth in Orlando, FL, is seeking a Registered Nurse for Palliative Medicine and Supportive Care. The role supports East Orlando and Altamonte practices, with a Monday–Friday schedule, day shift, and located at AHMG Palliative Medicine and Supportive Care.

The successful candidate will provide comprehensive nursing care, health education, and coordinate with a multidisciplinary team. Required RN license and BLS, Associate degree required; Bachelor's preferred.

Formación

• Registered Nurse (RN) license is required.
• BLS CPR certification required.
• Associate degree in Nursing is required; Bachelor's is preferred.

Responsabilidades

• Assesss patients' physical, psychosocial, cultural, and spiritual needs.
• Develop and implement care plans with the interdisciplinary team.
• Demonstrate competency in therapeutic and diagnostic interventions.
• Advocate for patient safety and rights.
• Provide patient and family education and evaluate learning.
• Encourage engagement in care through motivational interviewing and teach-back.
• Deliver relationship-based care with cultural sensitivity.
• Delegate tasks to non-RN personnel per policy.
• Coordinate and implement group visits for high-risk patients.
• Aid in care transitions including med reconciliation and service coordination.
• Participate in patient experience and satisfaction initiatives.

Job Description

AHMG Palliative Medicine and Supportive Care

2501 N. Orange Ave., Suite 235, Orlando, FL

Monday - Friday 8am - 5pm

position will support practices in East Orlando and Altamonte

• Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data.
• Develops and implements patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards.
• Demonstrates competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection.
• Advocates for and protects the health, safety, and rights of each patient.
• Provides patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching.
• Encourages patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology.
• Delivers relationship-based care, engaging patients in all aspects of care with respect for cultural differences and diversity.
• Delegates tasks to non-RN personnel in accordance with State Board of Nursing and hospital policy.
• Coordinates and implements group visits for identified high-risk patient populations.
• Aids in the care transition of high-risk patients, including medication reconciliation and coordination of services.
• Contributes to continuous process improvement by actively participating in patient experience and satisfaction initiatives.
