Job summary

AdventHealth in Orlando, FL is seeking a full-time Hepatology Nurse to support complex patient care and coordinate with the Liver Transplant team. This role involves ongoing assessment, plan modification, and collaboration with care providers to improve outcomes.

The nurse will assist patients with insurance referrals, referrals, and FMLA forms, requiring strong communication and teamwork in a fast-paced environment.

Qualifications

• Bachelor's degree preferred.
• Three years nursing experience in inpatient and/or outpatient.
• Experience with care of patients with liver/GI disease, organ failure or chronic illness.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Supports complex patient care issues and proactive care plans.
• Liaises with Hepatology patients, Liver Transplant Coordinator, and community care providers.
• Advocates for Hepatology patients as part of the Hepatology team, including assessment, planning, coordination, modification of treatment plans, and outcomes monitoring.
• Assists patients with non-medical needs related to care insurance/referral processes, forms, and disability/FMLA.
