Job summary

AdventHealth is seeking an experienced Registered Nurse to support the Palliative Medicine and Supportive Care team in Orlando, FL. The role involves delivering comprehensive nursing care, coordinating with the interdisciplinary team, and promoting patient-centered outcomes.

You will conduct assessments, develop care plans, and educate patients and families while upholding AdventHealth’s mission and values.

Qualifications

• Registered Nurse license required (RN).
• Associate degree in Nursing required; Bachelor’s preferred.
• Basic Life Support (BLS) certification required.
• 1+ medical office experience preferred.

Responsibilities

• Assess patients’ physical, psychosocial, cultural, and spiritual needs.
• Develop and implement care plans in collaboration with the interdisciplinary team.
• Demonstrate competency in therapeutic and diagnostic interventions (IVs, wound care, meds, catheterization).
• Advocate for patients’ health, safety, and rights.
• Educate patients and families; assess learning needs and evaluate teaching.
• Encourage patient engagement through motivational interviewing and self-management.
• Deliver relationship-based care respecting diversity.
• Delegate tasks to non-RN staff per state policy.
• Coordinate group visits for high-risk patients.
• Assist in transitions of care including med reconciliation and service coordination.
• Contribute to patient experience and satisfaction initiatives.

AHMG Palliative Medicine and Supportive Care

2501 N. Orange Ave., Suite 235, Orlando, FL

Monday - Friday 8am - 5pm

position will support practices in East Orlando and Altamonte

• Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data.
• Develops and implements patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards.
• Demonstrates competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection.
• Advocates for and protects the health, safety, and rights of each patient.
• Provides patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching.
• Encourages patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology.
• Delivers relationship-based care, engaging patients in all aspects of care with respect for cultural differences and diversity.
• Delegates tasks to non-RN personnel in accordance with State Board of Nursing and hospital policy.
• Coordinates and implements group visits for identified high-risk patient populations.
• Aids in the care transition of high-risk patients, including medication reconciliation and coordination of services.
• Contributes to continuous process improvement by actively participating in patient experience and satisfaction initiatives.
