Job summary

Kindred Hospital Philadelphia is a 52-bed long-term acute care hospital offering in-depth care for extended recovery. We partner with physicians and provide 24/7 clinical care, located near Lawncrest park.

The role conducts patient assessments, maintains relationships with referral sources, and coordinates pre-admission evaluations. It emphasizes confidentiality, regulatory compliance, and teamwork with clinical and operational staff.

Qualifications

• Associate or Bachelor’s degree.
• Degree in healthcare related field preferred.
• Two (2) years clinical patient care/treatment experience in healthcare field preferred.
• Healthcare professional licensure preferred.
• Registered Nurse, Licensed Practical Nurse, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Social Work license.
• Demonstrated understanding of Medicare, Medicaid, and other payers as related to LTACH benefits.
• Must read, write and speak fluent English.

Responsibilities

• Conduct patient assessment in accordance with company standards.
• Work closely with patients and their family members to educate them on the benefits of Kindred Hospital.
• Assist in conducting tours of the hospital with families and referral sources as needed.
• Facilitates the timely admission of patients from the referral sources; completes the Pre-Admission Clinical Evaluation (PACE) form; coordinates physician communication; identifies patient needs for equipment, supplies and consult services.
• Meets with management staff, participates in marketing meetings and serves on various internal committees.
• Maintains a state-of-the-art knowledge of areas of responsibility and develops a program of continuing education.
• Maintains all information in a manner that ensures confidentiality and meets regulatory requirements.
