Job summary

Lifepoint Rehabilitation is looking for a Clinical Liaison to join our team and support admissions to the acute rehab unit. You will coordinate intake, conduct on-site assessments, and advise patients and families on rehabilitation options to meet patient needs.

The role emphasizes collaboration with case management, payors, and hospital partners to ensure the right admissions and top-quality care, with a focus on improving community health outcomes.

Qualifications

• Applicants should have Clinical Licensure.
• Previous clinical experience required.
• Valid driver's license.

Responsibilities

• Builds and maintains referral relationships to drive admissions to the acute rehab unit.
• Assist and coordinate the intake and pre-admission screening process.
• Perform on-site clinical judgment to determine admission appropriateness.
• Communicates rehabilitation and facility options to patients and families and assesses needs.
• Secures information relating to patients' resources and benefits.
• Acts as initial contact for external case management and discharge planner personnel with payors and hospital facilities.
