Kindred Hospital Philadelphia – Havertown is a 66‑bed long‑term acute care facility offering 24‑hour clinical services and a 9‑bed Acute Rehabilitation Unit. Located in the medical district near Darby Creek and West Chester Pike Expressway, the hospital serves patients seeking extended recovery.
Job Summary
Conduct patient assessments in line with company standards to determine potential admission. Build and maintain relationships with referral sources and internal teams. Coordinate the transmission of clinical and benefits information for possible admissions, ensuring confidentiality and regulatory compliance.
Essential Functions
- Conduct patient assessments according to company standards.
- Educate patients and their families on the benefits of Kindred Hospital.
- Assist with hospital tours for families and referral sources as needed.
- Facilitate timely admission of patients from referral sources: identify required medical record information, complete the Pre‑Admission Clinical Evaluation (PACE) form, coordinate physician-to‑physician communication, and determine specific patient needs for equipment, supplies, and consult services.
- Participate in marketing meetings and serve on internal committees as assigned.
- Maintain current knowledge of responsibilities and pursue continuing education as needed.
- Perform duties in accordance with the Company’s Code of Business Conduct and applicable laws.
- Ensure all information is handled confidentially and meets regulatory requirements.

Qualifications
Education
- Associate or Bachelor’s degree.
- Preferred degree in a healthcare‑related field.
Licenses/Certifications
- Healthcare professional licensure preferred.
- Eligible licenses: Registered Nurse, Licensed Practical Nurse, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Social Worker.
Experience
- Two (2) years of clinical patient care/treatment experience in the healthcare field preferred.
- Demonstrated understanding of Medicare, Medicaid, and other payers related to LTACH benefits.
