This role combines hands-on patient care with sales, community outreach and referral development to support patient well-being and grow our home health services.ResponsibilitiesClinical Nursing ResponsibilitiesProvide skilled nursing care in accordance with physician orders and established care plans.Monitor patient conditions, assess pain and educate patients and families on care needs.Administer medications safely and follow established medication administration procedures.Promote patient safety, confidentiality and quality of care.Sales & Referral DevelopmentBuild and maintain relationships with healthcare providers, community partners and referral sources.Educate referral partners about Select Home Health services.Identify opportunities to expand referral networks and increase patient census.Execute outreach plans and track activities in the CRM.Represent Select Home Health at community events and networking opportunities.Monitor market trends and identify opportunities for growth.Administrative & Team CollaborationMaintain accurate records and complete required reports.Coordinate with clinical teams to support patient needs and referral transitions.Collaborate with leadership on outreach initiatives and agency growth.Ensure clinical and business development activities comply with applicable regulations and agency policies.Communicate patient status and needs with the interdisciplinary care team.Complete accurate and timely clinical documentation.Participate in required trainings, in-services and meetings.Follow agency policies, procedures and emergency protocols.Disclaimer:This description reflects the general nature and level of work for the Licensed Practical Nurse (LPN) / Account Executive position.
