Job summary

Select Home Health & Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to provide high-quality in-home nursing care in Carmel, IN. You will work as part of an interdisciplinary team, following physician orders, care plans, and agency protocols to ensure patient safety and comfort.

Responsibilities include monitoring patient status, administering medications, educating patients and families, and documenting care accurately.

Qualifications

• Graduate of an accredited nursing program.
• Active state LPN license in the state of Indiana.
• Recent acute care or home health experience (home health preferred).
• Valid driver’s license, reliable vehicle, and auto insurance.

Responsibilities

• Provide skilled nursing care per physician orders and care plans.
• Monitor patient condition, assess pain, and educate patients and families.
• Administer medications safely following the 7 rights.
• Ensure patient safety and maintain confidentiality.
• Communicate patient status and needs with the care team.
• Complete accurate and timely documentation.
• Participate in required trainings, in-services, and meetings.
