This role combines hands-on patient care with sales, community outreach and referral development to support patient well-being and grow our home health services.**Responsibilities*** **Clinical Nursing Responsibilities*** Provide skilled nursing care in accordance with physician orders and established care plans.* Monitor patient conditions, assess pain and educate patients and families on care needs.* Administer medications safely and follow established medication administration procedures.* Promote patient safety, confidentiality and quality of care.* **Sales & Referral Development*** Build and maintain relationships with healthcare providers, community partners and referral sources.* Educate referral partners about Select Home Health services.* Identify opportunities to expand referral networks and increase patient census.* Execute outreach plans and track activities in the CRM.* Represent Select Home Health at community events and networking opportunities.* Monitor market trends and identify opportunities for growth.* **Administrative & Team Collaboration*** Maintain accurate records and complete required reports.* Coordinate with clinical teams to support patient needs and referral transitions.* Collaborate with leadership on outreach initiatives and agency growth.* Ensure clinical and business development activities comply with applicable regulations and agency policies.* Communicate patient status and needs with the interdisciplinary care team.* Complete accurate and timely clinical documentation.* Participate in required trainings, in-services and meetings.* Follow agency policies, procedures and emergency protocols.**Disclaimer:**This description reflects the general nature and level of work for the Licensed Practical Nurse (LPN) / Account Executive position.
