Job summary

Piedmont Health Services, Inc. is seeking a Clinic Nurse (LPN) for the PACE Nursing team in Pittsboro, NC. The role supports comprehensive elder care through the PACE program, providing nursing care under the Nurse Manager and coordinating with a multidisciplinary team.

The successful candidate will have a Practical Nursing accreditation, an active NC LPN license, BCLS certification, and at least one year of LPN experience, preferably in ambulatory settings; bilingual skills are a plus.

Qualifications

• Education: Practical Nursing Accredited Program or equivalent.
• Licensure/Certification: Current & Valid NC Licensed Practical Nurse; Basic Life Support (BCLS).
• Experience: One year of general Licensed Practical Nurse experience, preferably within last three years.
• Preferred: Ambulatory health care setting experience; bilingual capability.

Responsibilities

• Uses the nursing process: assessment, planning, implementation, evaluation, and documentation of patient care and education.
• Coordinates services with the health-care team to provide complex patient care.
• Provides onsite and telephone triage.
• Assists with check-in when needed.
• Acts as a member of the interdisciplinary care team and participates in huddles.
• Performs other duties as assigned.

About Position:

• Work Location:163 Chatham Business Dr, Pittsboro, NC 27312
• Schedule: Monday through Friday, 8:00am to 5:00pm
• Travel: N/A

Duties/ Responsibilities -Utilizes the nursing process including: assessment, planning, implementation evaluation and recording of patient care and patient education. Provides support to providers by anticipating needs and providing assistance in the provision of care. Coordinates services with other members of the health-care team, internal and external, to provide complex patient care. Answers or returns phone calls. Provides onsite and telephone triage. Assists with the check in process when needed. Serves as a member of the interdisciplinary care team and participates in huddles. Performs other duties as assigned. Practices within the scope of the North Carolina. Nurse Practice Act and Piedmont Health Services’ policies, procedures, and standards. Performs all other duties, as assigned.

Qualifications -

• Education: Practical Nursing Accredited Program, License Practical Nurse Diploma or Certificate
• Licensure, Registry or Certification Required: Current & Valid NC License Practical Nurse; Basic Cardiac Life Support (BCLS).
• Experience: One year of general Licensed Practical Nurse experience preferably within the last three years.
• Preferred experience: Previous experience in an ambulatory health care setting; bi-lingual

Immunizations:Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.

Pay Range: $59,050 - $79,358(commensurate with years of experience)
