Job summary

Piedmont Health Services, Inc. is seeking a Clinic Nurse (LPN) for the PACE Nursing team in Pittsboro, NC. The role supports comprehensive elder care through the PACE program, providing nursing care under the Nurse Manager and coordinating with a multidisciplinary team.

The successful candidate will have a Practical Nursing accreditation, an active NC LPN license, BCLS certification, and at least one year of LPN experience, preferably in ambulatory settings; bilingual skills are a plus.

Qualifications

• Education: Practical Nursing Accredited Program or equivalent.
• Licensure/Certification: Current & Valid NC Licensed Practical Nurse; Basic Life Support (BCLS).
• Experience: One year of general Licensed Practical Nurse experience, preferably within last three years.
• Preferred: Ambulatory health care setting experience; bilingual capability.

Responsibilities

• Uses the nursing process: assessment, planning, implementation, evaluation, and documentation of patient care and education.
• Coordinates services with the health-care team to provide complex patient care.
• Provides onsite and telephone triage.
• Assists with check-in when needed.
• Acts as a member of the interdisciplinary care team and participates in huddles.
• Performs other duties as assigned.
