The Grove at Valhalla Rehabilitation and Nursing Center is seeking a Speech Language Pathologist (SLP) to provide and direct speech therapy services, including assessment, treatment planning, and program development for patients under physician orders.

Key duties include screenings, evaluation interpretation, scheduling, discharge planning, and staff education. Requires a Master's in Speech-Language Pathology and state licensure or eligibility, with a strong commitment to patient-centered care.

Formación

• Master's Degree in Speech-Language Pathology.
• Current state license or license eligible.
• Demonstrates good skills in all forms of communication.

Responsabilidades

• Provides Speech Therapy services to patients and initiates referrals when appropriate.
• Cconducts screenings of patients at regular intervals to determine need for intervention/treatment.
• Evaluates patients to obtain data for treatment planning and implementation.
• Interprets and communicates evaluation findings to patients, family, and team members.
• Establishes individual patient schedules in conjunction with other team members.
• Develops treatment plans with goals and methods to achieve them.
• Coordinates treatment plan with patient, family and team members.
• Monitors patient progress and modifies treatment as indicated.
• Recommends and obtains equipment to improve patient communication.
