Job summary

The Grove at Valhalla Rehabilitation and Nursing Center is actively seeking a Speech Language Pathologist (SLP) to deliver and oversee comprehensive therapy services. The SLP will assess, diagnose, and treat patients, develop individualized programs, and provide in-services as directed by physician orders.

Responsibilities include screenings, evaluations, scheduling, discharge planning, and collaboration with families and the care team.

Qualifications

• Master's Degree in Speech-Language Pathology.
• Current state license or license eligible.
• Demonstrates good skills in all forms of communication.

Responsibilities

• Provides and directs Speech Therapy services to patients, including assessment and treatment under physician orders.
• Conducts screenings of patients to determine need for intervention.
• Evaluates patients to obtain data for treatment planning and implements plans.
• Communicates evaluation findings to patients, families, and team members.
• Establishes patient schedules with the care team.
• Develops and updates treatment plans with goals.
• Coordinates plan with patient, family and staff.
• Monitors progress and adjusts treatment to achieve goals.
• Recommends equipment to improve communication and trains users.
• Participates in discharge planning.
• Develops home or community programs to maintain skills.
• Documents results of assessment, treatment, and follow-up.
• Supervises students and volunteers as required by state rules.
• Maintains continuing education and professional development.
