Job summary

The Grove at Valhalla Rehabilitation and Nursing Center is seeking a Speech Language Pathologist (SLP) to provide and direct Speech Therapy services. Responsibilities include assessment, treatment planning, and collaboration with physicians and the patient’s care team to support communication goals.

Minimum requirements include a Masters in Speech-Language Pathology and a current state license or license eligibility.

Responsibilities

• Provides Speech Therapy services to patients and initiates referrals.
• Conducts screenings of patients at regular intervals.
• Evaluates patients to obtain data for treatment planning.
• Interprets evaluation findings to patients, family, and team.
• Establishes patient schedules with team members.
• Develops treatment plans and goals.
• Coordinates treatment plan with patient, family and team.
• Monitors progress and modifies treatment as needed.
• Instructs on use of augmentative communication devices.
• Participates in discharge planning.
• Develops home/community programming to maintain function.
• Documents results and submits statistics.
