Job summary

Wyckoff Heights Medical Center is seeking an outpatient clinical nurse to oversee training for clinical staff in Partnership for Health interventions. You will support integration into clinical sessions, monitor fidelity, track service delivery and manage intervention data.

The role requires an Associate Degree or certification, NY LPN/RN license, and 1+ year in ambulatory care. You will provide patient-flow nursing support and lead the team in delivering prevention and adherence interventions.

Qualifications

Minimum formal education: an Associate Degree or certification equivalent.
Must be a Licensed Practical Nurse or Registered Nurse in NY with HIV Specialist certification.
At least 1 year in ambulatory care or specialty clinic setting.

Responsibilities

Oversee training for all clinical staff on PFH interventions and integration into clinical sessions.
Monitor fidelity, track service delivery and report data.
Coordinate booster trainings and manage intervention data.
Provide nursing support to manage patient flow and ensure optimum care delivery.
Deliver evidence-based interventions with fidelity on prevention, health education, and adherence.
Assist in facilitating access to medical and social services via case conferencing.
Enter and track patient data in EMR for PFH patients.
Lead daily huddles with the care team.
Promote safety culture through communication and teamwork.
