Job summary

Community Healthcare Network (CHN) is seeking a dedicated staff member to enroll and assess clients in the Treatment Adherence Program. You will maintain detailed records and keep the primary care provider informed about progress.

This role emphasizes outreach, education, and coordination with medical staff to improve access to care. The ideal candidate has a college degree, experience with HIV treatment updates, and strong communication skills.

Qualifications

• College degree or satisfactory equivalent of education and experience.
• Experience working with HIV and current with the latest HIV treatment advances.
• Bilingual Spanish is preferred; strong communication skills required.

• Recruits and enrolls clients who are in need of treatment adherence services.

• Maintains documentation of encounters and progress notes.
• Maintains confidentiality and privacy of client information.

Position Summary

Responsible for enrolling and assessing clients and providing services to those clients enrolled in the program. Maintains detailed and accurate records, including documenting all contact with each client, and keeping the client’s primary care provider informed of the client’s progress in the adherence program.

Duties And Responsibilities

• Assesses client’s needs and provides services accordingly.
• Coordinates treatment adherence services with medical providers and other relevant staff.
• Recruits and enrolls clients who are in need of treatment adherence services.
• Provides education and outreach to clients in both one-to-one and group format.
• Participates in the organization, implementation and monitoring of the Treatment Adherence Program, including attending meetings as appropriate, and collecting, reviewing and producing educational materials.
• Refers clients to other service providers at CHN.
• Improves access to primary care for treatment adherence clients.
• Provides appropriate documentation of encounters, case conferences, etc.
• Facilitates related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy.
• Maintains competency by attending workshops, conferences and seminars.
• Compliance with Employee Health Services.
• Performs other duties, as assigned.

Certificate/License Required

• College degree or satisfactory equivalent of education and experience.
• Experience working with HIV and current with the latest HIV treatment advances.

Knowledge, Skills And Abilities Required

• Computer proficient, with experience in WordPerfect, Word, Excel, and data entry.
• Strong verbal and written communication skills.
• Strong interpersonal skills.
• Ability to organize and prioritize.
• Interest in and conviction of purpose and aims for all programs in the clinic.
• Respect for the rights of individuals to make their own decisions about their medical care
• Ability to communicate easily and display a cordial manner towards individuals from a variety of socio-economic, cultural and religious background.
