Job summary

Wyckoff Heights Medical Center in Brooklyn, NY seeks a Program Manager to oversee grant-funded RAP and UND programs within Ambulatory Services. You will supervise Specialists and Peer Educators, provide direct case management to a small caseload, and coordinate staff training and quality improvement across HIV case management programs.

You will liaison with community partners and the Consumer Advisory Board, develop treatment plans, track outcomes, and report monthly to the Director.

Qualifications

• Master's degree in Social Work or equivalent required.
• Ability to work independently and manage a caseload.
• Experience with HIV care, behavioral health services a plus.

Responsibilities

• Oversees grant-funded RAP and UND programs and supervises staff.
• Provides direct service and case management to a small caseload.
