Job summary

Aurora Health Care seeks a Diabetes Educator to deliver DSMES and MNT services in alignment with ADA/ADCES standards. The role requires RN or RD licensure in Wisconsin and experience in patient education.

You will lead group and individual sessions, coordinate with the care team, and maintain CMS-compliant documentation and billing for DSMT and MNT services.

Qualifications

• CDCES preferred; certification within two to three years
• 2 years of patient education experience
• Strong knowledge of DSMES standards
• Ability to document and bill DSMT and MNT services

Responsibilities

• Provide care following DSMES standards per ADA/ADCES.
• Assess learning needs, goals, and barriers; tailor education accordingly.
• Lead individual and group diabetes education sessions.
• Provide formal diabetes management instruction (inpatient/outpatient).
• Provide medical nutrition therapy if RD.
• Offer ongoing support and follow-up for diabetes management.
• Collaborate with the healthcare team to develop care plans.
• May adjust diabetes medications under provider order per protocol.
• Stay current on ADA standards and diabetes care developments.
• Document and charge for DSMT/MNT services per CMS guidelines.
• Ensure age-appropriate patient education and growth/development awareness.
• Participate in community events and education program evaluation.
• Serve as a resource for providers, patients, and community.
• Complete required diabetes-related CE hours if not CDCES yet.

Major Responsibilities

• Provides care following the National Standard for Diabetes Self-Management Education and Support (DSMES) as defined by the American Diabetes Association (ADA)or Association of Diabetes Care and Education Specialist (ADCES).
• Provides care following the National Standard for Diabetes Self-Management Education and Support (DSMES) as defined by the American Diabetes Association (ADA)or Association of Diabetes Care and Education Specialist (ADCES).
• Assess patient learning needs, goals, and barriers to care, and tailor educational interventions accordingly.
• Lead individual and group education sessions on diabetes management, including medication, nutrition, exercise, and blood glucose monitoring.
• Provides formal instruction on diabetes management to the patient (inpatient and/or outpatient).
• If a Registered Dietitian, provide medical nutrition therapy services for patients with diabetes or other referring diagnosis
• Provide ongoing support and follow-up to ensure successful management of diabetes or other referring diagnosis.
• The educator will maintain knowledge of diabetes management technology and manufacturer protocols.
• Collaborate with the healthcare team to develop and implement comprehensive care plans.
• Under Provider order with proven competencies and within departmental policy, may adjust diabetes medications per protocol.
• Stay current on developments in diabetes care and education, ensuring compliance with ADA standards.
• Document and charge for services provided consistent with CMS and payor requirements and guidelines for DSMT and MNT services.
• Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span.
• Serves as a health resource to the community by participating in community events and coordinating support meetings. Participates in the development and evaluation of patient education program and materials.
• Acts as a resource for health care providers, patients andthe community.
• If a teammate does not yet have CDCES, must complete the required hours of diabetes-related continuing education annually, per ADA /ADCES requirements.

Licensure, Registration, And/or Certification Required

• Current license or certification to practice as a Registered Nurse (RN) or Registered Dietitian, in the State of applicable state. Certified Diabetes Care and Education Specialist (CDCES) preferred. If not certified, you must obtain certification within two years preferred but not to exceed 3 years.

Education Required

• Degree in Nursing, Dietetics, or other related disciplines that meet the certification eligibility requirements for CDCES. If nursing degree, BSN preferred. Master's degree in Dietetics required if Dietician license obtained after 01/01/2024.

Experience Required

• 2 years of patient-oriented educational experience preferred with a broad range of knowledge on diabetes for age-appropriate population and its interrelation to co-existing health conditions.
