Job summary

Serenity Home Health Care in Niles, IL is seeking a Client Wellness and Satisfaction Supervisor to lead home visits, orient clients to services, and assess care quality. You will supervise visits, document outcomes, and maintain strong client relations.

The role requires excellent communication, organization, and a valid driver’s license with insurance. You will coordinate with caregivers and manage client records in AxisCare, performing follow-up as needed.

Qualifications

• Ability to sit long period while driving.

Responsibilities

• Make home visits to new and existing clients.
• Greet and orient clients to the agency and explain scope of service.
• Conduct supervisory visits and assessments of care.
• Review and assess quality of caregiver care.
• Handle follow-up visits as needed.
• Prepare new client folders for visits.
• Document visits and client communications in AxisCare.
• Conduct client surveys every 3–4 months.
• Distribute necessary supplies to clients and caregivers.
• Perform other duties as assigned.
