Job summary

The Remarkable Agency is seeking a non-medical Case Manager to ensure families feel supported and informed throughout services. You will coordinate with families, service coordinators, and staff, schedule home assessments, and help match clients with appropriate Success Partners.

You’ll document progress, write clear assessment reports, and maintain smooth communication across teams while traveling for in-home visits within the service area.

Qualifications

• Strong written and verbal communication skills.
• Experience coordinating services for IDD populations.
• Comfortable using web-based software and email.

Responsibilities

• Make families and coordinators feel informed and supported.
• Schedule in-home assessments and coordinate visits.
• Draft assessments and recommendations for Regional Center coordinators.
• Match clients with suitable Success Partners to fit needs and schedules.
• Pre-frame first sessions to set expectations and reduce anxiety.
• Review daily notes to ensure accurate documentation.
